Asmanex Twisthaler (mometasone furoate) is an inhaled corticosteroid used to reduce the frequency and severity of asthma attacks. Note that this product will not help with an asthma attack in progress; rescue medications like epinephrine or albuterol should be kept on-hand and used as-needed.
This inhaler is a corticosteroid, meaning it's similar to cortisone. When applied to the lungs through inhalation, Asmanex Twisthaler prevents cells from releasing substances thought to contribute to asthma. It's a long term treatment that, over time, reduces the frequency with which asthma attacks occur and, when they do occur, they are often less severe.
Patient response to Asmanex Twisthaler as well as the degree of exposure to an allergen can influence results. Some individuals simply don't respond to inhaled steroids, and it's effect may be negligible if the patient is around a great deal of allergens---for example, someone who is allergic to horses will likely still have an asthma attack if going horseback riding. Others, however, find inhaled steroids are very beneficial and they can do things that they wouldn't be able to do without them.
Dosage & Administration
Dosage is best determined by a doctor, taking into account patient age, the severity of asthma, other medical conditions and so forth. Asmanex Twisthaler is available in 100, 200, and 400 mcg doses, meaning each inhalation delivers 100, 200, or 400 mcg to the lungs.
Once dosage is determined, treatment is almost always the same:
It takes one to two weeks of regular Asmanex Twisthaler use to see full benefit. If after that time period there's no significant improvement, dosage may be increased to the next level. If still no significant relief, an additional medication may be added. Again, some individuals simply don't respond to inhaled steroids at all; at some point, if no improvement is seen, patients may wish to stop inhaled steroids rather than risk side effects.
Many patients do notice improvements, though, and find they're better able to function with less dependency on rescue treatments. For these individuals Asmanex Twisthaler can significantly improve quality of life.
The inhaler will need to be primed before first use and if it's left unused for more than five days; to do so, simply spray it four times before use. Asmanex Twisthalers are metered, showing patients how many metered doses are left; after the meter runs out, there's no guarantee how much mometasone furoate will be delivered.
Precautions & Side Effects
Asmanex Twisthaler contains HFA as a propellant. Some individuals do not respond well to HFA, and may wish to avoid this product.
This product may contain lactose; if sensitive to it, patients may wish to avoid this product.
Asmanex Twisthaler delivers corticosteroids to the lungs; this is a steroid and may inhibit immune system response. Patients with an existing infection in the mouth, throat, nose, or lungs should not begin treatment with Asmanex Twisthaler. Patients who develop an infection while using the inhaler should get it treated quickly; if it's slow to respond to medications, use of Asmanex Twisthaler may need to stop.
While not as much of a concern as systemic steroids (administered by tablet or injection), growth suppression is a concern if Asmanex Twisthaler is used in children. Doctors may want to monitor this closely; be sure to attend regular doctor visits.
In otherwise healthy patients side effects are usually mild, though some may need to be addressed promptly if they do occur:
Women who are pregnant or breastfeeding should discuss risks versus benefits with a doctor before use. The medication can effect unborn babies and is known to be present in breast milk, and does pose a risk of prematurity, low birth weight, and similar conditions.
