Uloric (febuxostat) is used to treat high uric acid (a condition known as hyperuricemia) in patients who have gout. It is generally not used to lower uric acid levels in patients who do not have gout.

Uloric is a brand name used in the USA; it is also known by the brand name Adenuric in Europe.

Advantages and Disadvantages of Uloric

In treating gout Uloric is often compared to allopurinol, another widely-used gout treatment more commonly known by the brand name Zyloprim. In clinic trials and studies of real-world use, both of these medications have proven effective treatments but they also each have their pros and cons:

  • When standard doses are administered, Uloric has proven more effective than Zyloprim.
  • At standard doses, patients had positive results faster with Uloric than Zyloprim.
  • At higher doses, Zyloprim has proven more effective than Uloric.
  • Zyloprim is more versatile than Uloric; it can be used to manage uric acid levels when gout is not present.
  • The medications produce different side effects; individuals who experience a disruptive side effect from one may experience no side effects from the other.
  • Uloric is better tolerated by patients who have kidney problems.

What to Expect from Uloric

Uloric is administered by tablet, taken once per day with or without food as desired. Significant reduction in uric acid levels should be measurable after two weeks of treatment; if uric acid levels are not at desired levels after two weeks dosage may be increased and reevaluated after another two weeks.

It is common for patients starting Uloric (and other gout treatments) to experience gout flare-ups, which may continue for up to six months. This happens because as uric acid is reduced in the bloodstream it is mobilized from deposits in tissues, causing symptoms. In other words, it happens because the medication is working. During this time flare-up treatments like colchicine or NSAID painkillers, such as ibuprofen, may be used to manage symptoms. A doctor can recommend more powerful medications if needed.

Administration is simple:

  • Most patients are started on 40 mg per day, taken once per day.
  • Results should be evaluated after two weeks. If needed, dosage may be increased to 80 mg per day, taken once per day.
  • Results should again be evaluated after two weeks. At this point, if uric acid levels are still too high, some patients will have better results with Zyloprim. If Zyloprim cannot be taken, Uloric may be increased to 120 mg per day.
  • Treatment typically lasts a minimum of six months and may continue for several years if needed.

After the initial six-months of treatment, when flare-ups may be more common, most patient find flare-ups decrease in both frequency and severity, and many patients no longer have flare-ups at all. Of course, results will be best when lifestyle changes are also made to minimize the condition.

Annual checkups are recommended to ensure the medication is still working and to check for any developing side effects.

Precautions & Side Effects

Side effects from Uloric are typically mild and often stop occurring after the first few weeks of treatment. Most common are:

  • Nausea
  • Mild dizziness
  • Mild skin rash

Patients with other medical conditions are at greater risk of experiencing more serious side effects. Discuss risks and warning signs with a doctor before using Uloric if there is a history of:

  • Liver problems
  • Heart disease
  • Cancer
  • Organ transplant

Uloric should not be used while taking medications containing azathioprine or mercaptopurine.

The effects of Uloric on unborn or nursing babies are unclear; the medication should be avoided during pregnancy.
