Allylestrenol is a synthetic progestogen (called progestins) used in conception or pregnancy complications in women, when the complications result from low levels of naturally-occurring progesterone. It may also be used to treat benign prostatic hyperplasia in men.

Though widely used throughout much of the world since its introduction in the 1960s, allylestrenol remains a controversial treatment with conflicting results from both clinical trials and real-world use. This is likely because the conditions it treats are very complicated and usually involve numerous factors, which can produce erratic results with no clear explanation for the inconsistencies.

Allylestrenol is typically prescribed to:

  • Promote conception in women who have difficultly becoming pregnant
  • Reduce risk of miscarriage in women who have had two or more consecutive miscarriages
  • Prevent premature labor
  • Alleviate effects in certain other pregnancy complications
  • Reduce prostate size and alleviate symptoms of enlarged prostate

With regard to using allylestrenol to improve conception or prevent miscarriage, the most important thing to understand is that this medication is effective only when the complications result from low levels of progesterone. There are many other reasons that fertility may be impeded or spontaneous miscarriages occur. Laboratory tests are the only way to evaluate progesterone levels.

Allylestrenol for Conception

Progesterone is one of the most critical fertility and pregnancy hormones. During monthly cycles it prepares the uterus for the implantation of a fertilized egg; if fertilization does not occur, progesterone levels will drop---resulting in menstruation. If levels are too low implantation will not occur or the zygote will quickly be rejected.

If low progesterone levels are suspected in preventing pregnancy allylestrenol may provide benefit; it mimics the effects of progesterone, preparing the uterus for pregnancy.

Dosage will depend on the severity of the hormone deficiency, which can only be determined with laboratory tests. Dosage typically ranges from 10 to 40 mg per day, divided into two or three doses taken throughout the day. Treatment typically lasts for one week at a time, but again this can vary.

Allylestrenol during Pregnancy

Progesterone not only prepares the uterus for conception; it remains critical throughout pregnancy, inhibiting the immune system from rejecting the fetus, promotes nourishment, prevents further ovulation, and promotes growth of glands needed for milk production. If levels of the hormone are too low the uterus can become inhospitable to the fetus, resulting in miscarriage or premature labor.

Allylestrenol may be helpful if low progesterone is suspected as a cause of prior miscarriages or if low hormone levels are detected during pregnancy, causing concern for premature labor. Low progesterone is just one of many reasons miscarriages or premature labor may occur, however, and again it's critical that hormone levels be checked prior to beginning treatment---too much of the hormone can be every bit as harmful as too little.

Dosage during pregnancy should only be determined by a doctor; there are simply too many variables to generalize. Similar to use in conception, treatment often lasts for only a week at a time, though in some scenarios it may be longer.

Allylestrenol in Benign Prostatic Hyperplasia

Benign prostatic hyperplasia simply refers to non-cancerous growth of the prostate, which can lead to difficulty urinating or completely emptying the bladder, which in turn can lead to more complications, such as bladder stones, urinary tract infections, and kidney damage.

Though a relatively new application for allylestrenol, it has proven beneficial in both clinical studies and real-world use for alleviating symptoms, preventing further growth, and reversing growth. Results do, of course, vary between individuals, and there are other medical factors that may also influence effectiveness.

For benign prostatic hyperplasia dosage is typically 50 mg per day, and treatment may continue for months at a time. Allylestrenol, like any hormone, is not without risks; regular doctor checkups are strongly encourage for the duration of treatment to ensure the medication is working as intended and no complications are developing.
