Cafergot (ergotamine tartrate, caffeine) is a dual-action medication used as an abortive treatment for migraine headaches. Some patients also find it effective for cluster headaches, but many find alternative delivery systems -- injections or nasal sprays -- are more effective for cluster headache pain. 

Cafergot employs both ergotamine and caffeine to constrict blood vessels in the head, which reduces migraine pain and symptoms, though the exact mechanism behind migraine headaches is unknown. Both ingredients also interfere with pain receptors, dulling the sensation of pain. Together ergotamine and caffeine enhance each other, though ergotamine is considerably more powerful than caffeine. 

How to Stop a Migraine 

An abortive medication stops a headache from fully developing; it doesn't just mask pain. In order to stop a headache from developing, it must be administered before the headache gets very far along. Conversely, it won't prevent a headache from starting in the first place; there has to be a headache there to stop. 

This means there is a small window of opportunity which is the ideal time administer Cafergot: Immediately after the first symptoms begin. 

Most migraine sufferers have warning signs that a headache is coming on; auras, nausea, sensitivity to light or noise, neck pain and so forth. Unfortunately many cluster headache sufferers do not have any warning other than severe pain, which may be why the faster delivery systems of injections or sprays are preferred by so many. 

When warning symptoms are present, they should be considered the start of a headache, and Cafergot should be administered as soon as possible: 

  • Take 2 tablets, or 2 mg, at the earliest symptoms of a headache 
  • If headache persists, take an additional tablet, or 1 mg, after 30 minutes 
  • Continue taking an additional tablet every 30 minutes, up to 6 tablets total, until symptoms are gone 
  • Do not exceed 6 tablets in 24 hours or 10 tablets in 7 days 
  • Overuse may result in rebound headaches, increases risk of adverse effects, and reduces effectiveness of the medication 
While most effective if taken at earliest headache symptoms, Cafergot may still provide some degree of relief when taken at later stages. 

Some patients find effectiveness diminishes over time even without overuse. Likewise some patients find Cafergot is not very effective to begin with; as with any medication, reactions vary. There are numerous similar migraine and cluster headache treatments available, with different active ingredients and delivery methods, which may prove more helpful if Cafergot isn't providing sufficient relief. Speak with a doctor about alternative options -- relief is out there. 

Warnings & Adverse Effects 

As previously mentioned, this medication works by constricting blood vessels. The vessels which seem to impact migraines and cluster headaches are in the head, but the vasoconstriction effect of Cafergot takes place throughout the body. Patients with blood circulation problems should use it with caution, as it may exacerbate the condition. 

Most side effects are rare and not cause for alarm, but a few do call for emergency attention if experienced: 
  • Sudden, severe headache 
  • Numbness or weakness 
  • Confusion, difficulty speaking 
  • Rapid or very slow heart rate 
  • Painful cough 
  • Difficulty breathing 
  • Dangerously high blood pressure 
Some patients are at greater risk of experiencing severe side effects, or should avoid using this product altogether: 
  • Use with caution if peripheral vascular disease, hypertension, heart disease, impaired hepatic or renal function, or sepsis are present 
  • Patients taking protease inhibitors, macrolide antibiotics and other CYP 3A4 Inhibitors are at risk of severe reactions and should avoid using this product altogether
  • Cafergot will have no effect on other types of headaches
  • This product may be harmful to unborn and nursing babies

This medication should not be used in patients under age 18 or over age 65 without explicit doctor approval; there is a significantly higher risk of side effects.

Managing Migraines with Imigran

Imigran is predominately used to treat migraines, particularly in tablet form. It alleviates both pain and other symptoms, such as auras, nausea, and sensitivity to light and sound. Treatment is simple and straightforward:

  • Take one dose of Imigran at the earliest symptoms of a migraine.
  • If pain and/or other symptoms are not sufficiently alleviated, or the headache returns, a second dose may be taken after two hours.
  • Do not exceed 300 mg in a 24 hour period.
  • Tablets can be crushed, chewed or dissolved, but they are very bitter. Those who have difficulty swallowing tablets may want to consider Imigran Radis, which is designed to be dissolvable (but will still taste bitter).

Patients are usually started on 50 mg doses. Some individuals who have particularly severe migraines or who are simply less sensitive to Imigran may find 100 mg more effective. As with any medication, however, there is a risk of side effects and this risk is higher with higher doses; the lowest effective dose is preferable.

This medication tends to produce the best results if taken at the earliest signs a headache is developing, but Imigran will still provide benefit if taken at later stages. Effects start to be felt about 30 minutes after taking a dose.

For unclear reasons some patients simply don't respond to Imigran. If there is no improvement at all after the first dose, a second dose should not be taken. A doctor or pharmacist can suggest alternative options.

Imigran for Cluster Headaches

Triptan medications are one of the few effective options out there for cluster headache-suffers. With cluster headaches, taking the medication at the earliest symptoms is very important; the medication may not provide significant benefit if taken at later stages.

Because the timing is so critical with cluster headaches, tablets---which take about 30 minutes to start working---are usually not the ideal choice. They do work for some individuals, however; treatment is the same as for migraines (above).

Faster-acting Imigran options are:

Many patients are understandably averse to injections, but it is the most effective route for rapid pain relief in the majority of cluster headache-sufferers. It's also not as bad as many people imagine---it is injected subcutaneously (under the skin) rather than into muscle or the bloodstream; needles for this purpose are very short and thin, virtually painless.

Warnings & Side Effects

Patients with certain health conditions are at greater risk of experiencing complications when taking Imigran. Discuss risks with a doctor before use if there is a history of:

  • Poor blood circulation
  • Coronary heart disease
  • Angina
  • Seizures
  • High blood pressure
  • Heart attack or stroke
  • Liver or kidney impairment
  • Ischemic bowel disease

As with any medication there is a risk of side effects when using Imigran. Most are mild and pass quickly, including:

  • Flushing or feeling warm, particularly in the chest area
  • Mild dizziness or lightheadedness
  • Nausea or vomiting
  • Muscle cramps
  • Drowsiness

The above side effects are not cause for concern unless severe enough to be disruptive.

Like all triptan medications, Imigran Nasal Spray works by narrowing blood vessels in the head. During migraine or cluster headache episodes blood vessels on the surface of the brain become swollen; while this is not the only change that occurs and may not be the only factor contributing to pain, it has become clear through decades of widespread triptan use that the narrowing of these blood vessels significantly alleviates or eliminates pain during these types of headaches. Auras and other symptoms associated with migraines, such as light sensitivity and nausea, are also often alleviated. Because of the way this medication works---it is not a painkiller---it will have no impact on other types of headaches, such as tension or sinus headaches.

Using Imigran Nasal Spray

Imigran Nasal Spray is available in two strengths; 10 mg and 20 mg. This is how much medication is delivered in each dose (spray). The standard dose that most patients use is 20 mg, though some find 10 mg sufficient. The lowest effective dose is preferred, as risk of side effects does increase with higher doses.

The medication is easy to use:

  • Administer one dose at the earliest symptoms a headache is developing.
  • If symptoms return, an additional dose may be administered as long as there is at least two hours between doses.
  • Do not exceed 40 mg of Imigran Nasal Spray in a 24 hour period.

Effects start to be felt about 15 minutes after administration. It is not unusual for other medications, such as traditional painkillers like ibuprofen or other NSAIDs, to be taken along with triptan medications during a headache episode. Patients are strongly advised to check with a doctor before doing so, particularly if there are any major medical conditions present.

Other Imigran Options

Imigran is available in a variety of formulations. Cluster headache-sufferers in particular should pay attention to the speed at which the medication begins to work; this is often a critical factor in effectiveness.

  • Imigran Tablets - the slowest-acting form of the medication, effects are felt about 30 minutes after administration. Tablets are generally not the best choice for most cluster headache-sufferers.
  • Imigran Radis - these tablets are designed to be dissolved in fluids. Effects are felt slightly faster than regular tablets. Ideal for patients who have difficulty swallowing whole tablets, though it will taste bitter.

  • Take one dose of Imigran Radis at the earliest signs a headache is starting. Tablets can be swallowed whole or dissolved in fluids or wet foods, such as applesauce.
  • If pain and/or other symptoms are not sufficiently alleviated, or the headache returns, a second dose may be taken after two hours.
  • Do not exceed 300 mg in a 24 hour period.

Avoid mixing this medication with other drugs without first speaking to a doctor or pharmacist about potential interactions; resulting side effects can be serious. Of particular concern is SSRI, SNRI, and MAOI medications, which are usually prescribed to treat depression or similar mental illnesses.

While Imigran Radis works slightly faster than regular Imigran tablets, it is not the fastest delivery method. Some individuals---particularly those who experience cluster headaches---find faster methods to be considerably more effective. Alternative options are:

This medication is, as its name suggests, administered by injection. There are two purchasing options:

  • The starter pack includes 2 cartridges and a reusable pen injector.
  • The refill pack includes 2 cartridges, which fit into the pen injector.

Each cartridge contains 6 mg sumatriptan, equivalent to one dose.

Many patients are understandably averse to administering medication via injection. This product, however, is delivered subcutaneously---just beneath the skin, rather than into muscle or veins. Needles used for subcutaneous injections are very short and fine. Taking Control with Imigran Subject Injection

Imigran Subject Injection is particularly effective in cluster headaches, and individuals who experience particularly severe migraines also tend to prefer injections over slower-acting formulations.

Standard treatment is simple:

  • Administer one dose (6 mg, one cartridge) at the first symptoms of a headache, or as soon as possible if unable to do so at the headache's start.
  • If symptoms return a second dose may be taken after two hours.
  • Always wait at least two hours between doses.
  • Never exceed 12 mg (2 doses) in a 24-hour period.

Effects are felt within 5 to 10 minutes of administration. Side effects from Imigran Subject Injection, if experienced at all, are usually mild and pass quickly. Common side effects include:

  • Flushing or feeling warm, particularly in the chest area

Sumatriptan is the generic name for the first migraine medication in the triptan class of drugs. Available in a range of dosage and administration methods, Sumatriptan is available in tablets, nasal spray and by injection. The product discussed on this page is sumatriptan tablets.

This medication is useful for alleviating migraine pain and related symptoms, such as light sensitivity, auras, or nausea. It does this by attacking a migraine on two fronts:
  • Constricts swollen blood vessels in the brain, thought to be the ultimate cause of migraines
  • Blocks pain signals from being transmitted
Sumatriptan is also sold under the brand name Imitrex, and is similar to other triptan migraine treatments such as Migrex and Zomig.

Migraine Relief with Sumatriptan:

No one knows exactly why some individuals experience migraine headaches, and there is no cure. Sumatriptan is an abortive medication, which means it stops a headache in progress. It has no effect if taken prior to a headache beginning -- it will not prevent a migraine or lessen the frequency with which migraines occur.

Sumatriptan works best if taken when the first signs of a migraine are noticed, such as auras. Some migraine sufferers have no warning; the medication will still help ease the pain even if taken at later stages.

Ideally it is taken with plenty of fluids, which aids absorption into the bloodstream. It may be taken with or without food.

Effects are felt about 30 minutes after administration.

If one dose does not eliminate the migraine or reduce symptoms to tolerable levels, another dose may be taken after an interval of 2 hours. 200 mg of this medication should not be exceeded in a 24 hour period without explicit directions from a physician.

Triptans and Cluster Headaches:

Triptans have proven effective at reducing pain in the difficult-to-treat cluster headache; however, tablets are the slowest and least effective form of administration for this purpose.

Cluster headache sufferers seldom have any warning that a headache is about to begin. Given the severity of the pain involved in a cluster headache, fast relief is imperative. For cluster headaches, the fast-acting Imitrex Injections or only slightly slower Imitrex nasal spray is better option.

Though Sumatriptan is effective against migraines and cluster headaches, it will not significantly reduce pain or symptoms in any other types of headaches.

Precautions and Warnings:

Sumatriptan is a sulfa drug and should not be used by individuals who are hypersensitive to sulfa. Symptoms of an allergic reaction include swelling of the tongue, lips or eyes, or swelling of the throat.

Like most medications, Sumatriptan may interfere with the effectiveness of other prescription drugs or cause unexpected side effects. Never mix medications without first speaking to a pharmacist or physician.

Fewer than 10% of Sumatriptan users experience mild side effects, including:
  • Fatigue
  • Warm or cold flashes
  • Vertigo
  • Nausea, vomiting or diarrhea
  • Muscle cramps
  • Sleepiness
Less frequently more serious side effects are experienced. Medical attention should be sought if experiencing:
  • Chest pain
  • Rapid heartbeat
  • Difficulty breathing
  • Vision changes
  • Severe stomachache
Individuals who have a history of heart, liver or kidney problems have a higher risk of experiencing side effects when taking this product.

Taking too much Sumatriptan may cause a serious heart condition. Even if no symptoms are noticed, overdose of this medication warrants emergency medical attention.

Avoid taking other triptan medications in conjunction with Sumatriptan, including other forms of administration such as nasal spray, without first speaking to a doctor about potential risks.

Other Considerations:

Some individuals find Sumatriptan gradually becomes less effective at combating migraines. If this happens, speak with a doctor about alternative treatments. Sometimes alternating between treatments, even with very similar ingredients, will preserve or restore effectiveness.

  • Take one dose (50 mg, one tablet) at the earliest symptoms of a migraine.
  • If pain and/or other symptoms are not sufficiently alleviated, or the headache returns, a second dose may be taken after two hours.
  • Tablets can be crushed, chewed or dissolved, but they are very bitter. Those who have difficulty swallowing tablets may want to consider Imigran Radis, which is designed to be dissolvable (but will still taste bitter).

Most people use Imigran Recovery without any adverse reactions. Some patients, however, are at greater risk of experiencing complications from using this medication. Discuss potential risks with a doctor prior to using Imigran Recovery if there is any history of:

  • Any heart problems
  • Stroke
  • Angina
  • Vascular disease
  • Liver or kidney impairment
  • Severe or untreated hypertension

As mentioned above, Imigran Recovery is known to produce negative effects when mixed with some other medications. Do not use this product if also taking:

  • SSRIs, SNRIs, MAOIs and some other antidepressants
  • Medications containing ergotamine
  • Other medications containing triptans
  • St John’s Wort

The effects of Imigran Radis on unborn or nursing babies is unclear; use should be avoided while pregnant or nursing unless explicitly instructed otherwise by a doctor.

Other Imigran Formulations

Imigran offers a range of products containing the same active ingredient as this one. The speed at which the medication begins to work is an important consideration for those who suffer from cluster headaches or severe migraines; faster-acting typically means better results. The full line of products can be found below:

  • Imigran Tablets - effects are felt about 30 minutes after administration. Available in 50 mg and 100 mg doses.
  • Imigran Radis - similar to regular tablets, this formula is designed to be dissolved in fluids. Effects are felt slightly faster than regular tablets. Ideal for patients who have difficulty swallowing whole tablets.
  • Imigran Nasal Spray - effects are felt about 15 minutes after administration. This is sometimes a suitable alternative to injections for cluster headaches.
  • Imigran Subject Injection - starts to work 5 to 10 minutes after administration. Ideal for cluster headaches and severe migraines that react better to fast-acting treatment.

Some individuals simply don't respond to Imigran. If no effects are felt at all from Imigran Recovery, consult a doctor about alternative treatments. There are many out there!
