Acupil H (quinapril, HCTZ) is a combination medication predominantly used to help manage high blood pressure, though it is also sometimes used to help treat heart failure. Less commonly it may be used off-label in other conditions.
The two active ingredients in this product are:
Combined the two ingredients are a potent antihypertensive treatment. Patients are often started on a single medication to lower blood pressure, usually a diuretic, and only moved to a combination if a single medication is ineffective on its own. Doctors do sometimes start a patient on a combination product if hypertension is severe enough, but in general patients should not start with this medication unless explicitly prescribed.
Acupil H comes in tablets, usually taken just once per day. Food is optional. Once an effective dose is found, Acupil H may be taken for as long as it provides benefit, which often spans years and may extend to the remainder of life if other measures to manage hypertension are not utilized.
The typical starting dose of Acupil H is 10/12.5 mg quinapril/HCTZ, which may be increased up to 20/25 mg if needed.
Treatment should be individualized, taking into account numerous personal variable, including patient age, weight, race, other medications being taken and so forth, as well as patient response to treatment. Black patients often find hypertension is less responsive to medications than in other races, and may require slightly higher doses.
It takes about two weeks from the start of treatment or following dosage adjustments before maximum results are seen. Hypertension rarely produces significant symptoms until something catastrophic happens, so patients may not feel different, but it should be measurable.
Acupil H can interact with other, unrelated medications, including some over-the-counter varieties; double-check with a doctor or pharmacist before taking other treatments with this product.
Patients with hypertension, and sometimes those with heart failure, are usually also encouraged to make lifestyle changes when prescribed Acupil H. Changes usually include:
The above, if made and maintained over the long term, are often enough to manage blood pressure and may ultimately lead to the discontinuation of medication. Unfortunately the above are not always enough, or may be too difficult for a patient to acheive. These individuals will likely need to take Acupil H or a similar medication for the remainder of life.

The above side effects are generally not cause for concerns unless severe enough to be disruptive, with the exception of chest pain, which always warrants medical attention. In most cases side effects will stop occurring with the first few weeks of treatment, as the body adjusts. If they do not, consult a doctor.
