Co-codamol (paracetamol and codeine) is a compound analgesic which contains both paracetamol and codeine. It is typically taken as-needed to treat mild to moderate acute pain when paracetamol alone is insufficient.

This combination of drugs is widely used around the world and generally considered safe as long as it is not taken in large doses or for long periods of time.

Pain Relief When you Need It

Like most acute pain treatments, co-codamol is taken as-needed. It is commonly used to manage pain from:

  • Toothache or dental procedures
  • Menstrual cramps
  • Headaches, including migraines
  • Muscle cramps, sprains or other injuries
  • Minor medical procedures
  • Wounds or broken bones

In patients 12 years old and up, administration is straightforward:

  • Take 1 to 2 tablets as-needed every 4 to 6 hours.
  • The initial dose is ideally taken at the first symptoms of pain.
  • Taking with food and water is optional.
  • Do not exceed 8 tablets (4000 mg paracetamol) in 24 hours.

Length of treatment can vary; it may be taken just once or twice a month for conditions such as headaches; in other cases it may be taken for days straight to manage more persistent pain, as from toothache. When taken consecutively, most doctors do not advise using the medication for longer than three days, for a few reasons:

  • The longer it is taken the greater the chances of developing side effects;
  • Taking codeine long-term may lead to tolerance, in which the medication no longer works as well at lower doses;
  • Codeine can be habit-forming if used often, leading to irritability and anxiety when treatment ends.

If pain treatment is needed for longer than three days, staggering co-codamol with another painkiller will likely avert all three issues.

Other Considerations

Too much paracetamol can cause serious liver damage; be careful to not take more than the recommended dose of co-codamol. Also note that a great many other products, including over-the-counter treatments, contain paracetamol; read labels carefully before taking additional medications.

The medication should not be given to children under age 12 without doctor approval; dosage strength will likely be determined by weight. Too much codeine can produce serious, even life-threatening adverse reactions in young patients. Codeine is known to pass through breast milk; mothers who are breast-feeding should not take co-codamol.

Patients with any serious preexisting conditions should speak with a doctor about risks before using codeine products, particularly those with any sort of breathing disorder--opiates are known to depress breathing in patients with asthma, sleep apnea, and similar conditions.

Speak with a doctor if taking any other medications; co-codamol may interfere with their effectiveness or cause unanticipated side effects.

Because codeine often produces feelings of euphoria or well-being, it is a widely-abused drug. For this reason there may be regulations regarding its purchase or possession in some areas; clients are responsible for complying with any such applicable regulations.

If addiction does occur, treatment may be gradually tapered off. It is unlikely to produce physical withdrawal symptoms unless it has been used in very high doses for months or longer. A doctor can provide more advice for quitting codeine.

Zapain is available in Tablets or Capsules

Zapain (codeine phosphate, paracetamol) is an opioid pain relief treatment used as-needed to manage mild to severe pain.

Codeine and paracetamol are found in quite a few products, in varying ratios and strengths. Zapain provides one of the stronger doses available, with 30 mg codeine and 500 mg paracetamol. Another advantage this product has over competing brands is the individual packaging of tablets in blister packs, which keeps the medication fresher, and therefore more effective, than when stored loose in bottles.

Taking Zapain for Pain Relief

Using this treatment to stop pain is very simple:

  • Patients over 18 years of age should take 1 or 2 tablets every 4 hours as needed for pain relief.
  • Patients between 12 to 18 years of age should take 1 or 2 tablets every 6 hours as needed.
  • This product should not be used in patients under 12 years of age unless prescribed by a physician.
  • Patients of any age should not exceed 8 tablets, or 240 mg of codeine, in one day.
  • Tablets should be taken with a full glass of fluid.
  • Taking tablets with food may help ease nausea.

Zapain is typically used short-term, for a few days or weeks, or intermittently, such as for periodic migraines. Long-term, frequent use of the product often leads to tolerance, making the medication less effective unless taken in higher and higher doses. Chronic use of high levels of this product can lead to dangerous side effects, including liver and kidney damage.

If tolerance does develop, switching to an alternate medication for a few weeks will restore Zapain's effectiveness.

Warnings Before Use

One of the ingredients in this product, paracetamol, can cause serious liver or kidney damage if taken in high quantities. Be aware that many over-the-counter products, particularly other painkillers and cold or flu medications, also contain paracetamol. Alcohol consumption during treatment will increase the risk of liver damage. If too much paracetamol is consumed, seek medical attention even if no symptoms are present.

Patients with any major medical conditions should seek the advice of a doctor before taking Zapain. Likewise, check with a doctor or pharmacist before mixing medications; this product is known to interact with other prescription and non-prescription drugs.

Patients with a breathing disorder such as asthma or COPD, or a current lung infection, should use with caution; codeine may suppress breathing, particularly in higher doses.

Pregnant or breast-feeding women should discuss alternative pain therapies with a doctor; it is unknown if this product will harm unborn or nursing babies.

Long-term use of codeine may be habit-forming. For this reason possession and use of the product may be restricted in some areas; clients are responsible for complying with local regulations. Withdrawal symptoms include irritability, restlessness, insomnia and anxiety. Tapering off treatment by slowly reducing the amount of medication taken will help ease withdrawal symptoms.

Side Effects

Some side effects with this medication are fairly common, but generally not cause for concern. Most patients experience only one or two side effects:

  • Headache
  • Stomach upset
  • Abdominal pain
  • Flushing
  • Sweating
  • Sleepiness
  • Dizziness
  • Disorientation
  • Loss of appetite
  • Loss of libido
  • Mood changes
  • Low blood pressure

Because this product lowers blood pressure, individuals taking other drugs to lower blood pressure are at increased risk of dizziness, disorientation and fainting. Discuss risks with a doctor before use.

I agree with those... who say it seems more effective than what is sold in the local pharmacy--it is definitely fresher and packaged better and that may have a lot to do with it. I would also be interested in a lower dose of acetaminophen although if used as directed there as not much danger, unless you also do a lot of drinking. (Never a good idea to mix booze & paracetamol.) Kiwi has also been very responsive to all my questions. Great product and a class act pharmacy!

Quite perfect. This might not be a big deal to some, but the packaging is impressive. The box is very clearly labeled with all the drug information and the blister packs are easy to open with a finger... nail. The drug itself is perfectly formulated. I just take it if I have a seizure in the middle of the night and am feeling achy in the morning. It's not extreme pain so I've only taken 1 or 2 pills at a time so I can't speak on how the medicine relieves higher pain levels than that. For those who ask why it has so much acetaminophen, it is a calculated business decision which I don't think is appropriate for me to get into on this site. I recommend researching the topic instead of asking kiwi to carry codine without the acetaminophen.

Zapain, in the blister packs (as sold here), is the strongest codeine preparation I've ever used. One pill you get a full 30 mg. worth - fresh as the moment it was made - as opposed to a half-degraded..., crumbling bottled pill that's supposed to give you 30 mg. codeine. Fresher is better. I also like the 100-pill bulk. Going to my local doctor gets me 50 at a time (loose, in a poorly sealed bottle). So I buy it here, even though it costs me a little more.

The comparable local product is Tylenol 3 (or, as most of us get, the generic APAP with codeine 300/30). Let me tell you that Zapain is markedly superior. I don't know why, because it's really just a ...slight bit more acetaminophen than the local analog. When I take Zapain both ingredients come across full strength. The difference is so huge (from American Tylenol 3) that I've ordered Zapain 3 times from KiwiDrug now. I commend the manufacturer, Bristol Laboratories, U.K., for high quality and KiwiDrug for selling it. I just wish that express shipping was an option.

Speaking from experience Zapain is very high quality. I think the 500/30 formulation is perfect (American Tylenol 3's are 300/30). But more importantly Zapain tablets have a nice hard coating. Which m...eans it doesn't dissolve in your hand or 'go bad' easily. It comes in blister packs, which further guarantees the 'freshness' & protection against ambient air moisture. Thus, when you're taking these (I need to take 2), you're getting the absolute maximum of the drugs' intention. It takes about an hour to work, probably because of the tablet's hard shell, but take effect it does! For me it works better than "Lortab" & Vicodin, even though the medicine in those is supposed to be stronger. I find that Zapain works far better. Oh, and KiwiDrug ships very fast. I commend them for being on the ball with that.
